This policy explains how Fresh Start Health Centers ("Fresh Start," "we") handles information collected through hepcnow.com, our hepatitis C telehealth website. Fresh Start operates four Rural Health Clinics in Ashland, Grayson, Morehead and Vanceburg, Kentucky.
Information we collect on this website
- Information you submit. When you use our eligibility form, we collect the details you enter, such as your name, phone number, email address, state and how you heard about us. Please do not enter medical details in the form.
- Phone calls. If you call us, we receive your phone number and the information you choose to share.
- Website usage. We use Google Analytics to understand how visitors use this site, for example which pages are viewed and whether the form or phone link is used. Google Analytics uses cookies and receives information such as your browser type, device and approximate location. We do not use the Meta (Facebook) Pixel on this site.
- Security. Our hosting provider, Cloudflare, processes technical information such as IP addresses to protect the site from automated attacks.
How we use it
- To contact you about hepatitis C testing and treatment, verify your insurance and schedule care.
- To operate, secure and improve this website.
- To measure, in aggregate, whether our outreach and advertising reach people who need care.
How we share it
We do not sell your information. We share it only with service providers that help us run this website and respond to you, such as our form and contact-management provider, our website host and our analytics provider, and only as needed for those purposes. We may also disclose information when required by law.
When you become a patient
Once you become a Fresh Start patient, your health information is protected health information under HIPAA. It is handled as described in our Notice of Privacy Practices below, not under this website policy.

Our Notice of Privacy Practices describes how your medical information may be used and disclosed and how you can access it. You can request a copy at any Fresh Start clinic or by calling (606) 225-8200, and we will provide it to you on paper or by email.
Your choices
- You can ask us to stop contacting you at any time by calling us at (606) 225-8200. If you agreed to text messages, reply STOP to any text to opt out.
- You can block or delete cookies in your browser settings. You can also opt out of Google Analytics with Google's browser add-on.
